A: We provide individualized treatments to improve your specific needs including pain control, strength/flexibility
training,functional training, balance training, relaxation and education to continue your wellness program.
Q: HOW DO I SCHEDULE MY FIRST APPOINTMENT?
A: Appointments can be made by calling our front office. All new clients must bring completed forms
and documents on or before the first appointment.

Q: DO I NEED A DOCTOR'S REFERRAL BEFORE MY APPOINTMENT?
A: A doctor's referral for physical therapy is required to file insurance claims for all dates of service.
Q: HOW DO I CANCEL OR RESCHEDULE MY APPOINTMENT?
A: Appointments can be changed by calling the front office. Any changes must be completed 24
hours before the scheduled appointment time to avoid a $50 cancellation fee per company policy.
Q: IS MY INFORMATION SECURE?
A: We assure privacy of client information and adhere to the HIPPA Privacy Act.
Q: HOW DO I PROVIDE COMMENTS/SUGGESTIONS?
A: INPUT comment forms are located at the front desk and provided to our clients at each visit. Clients are also
encouraged to e-mail comments and suggestions to info@inshapesc.com. We value your input and aim to

Q: WHAT IS PHYSICAL THERAPY?
A: Physical Therapy is a skilled profession performed by licensed physical therapists and physical therapy
assistants in order to educate and rehabilitate individuals to improve their health and quality of life.
Q: WHAT IS AQUATIC THERAPY?
A: Aquatic Physical Therapy is a skilled technique performed in water by licensed physical therapists and physical
therapy assistants utilizing the unique properties of water. We offer individualized services in our tri-depth
performance pool with underwater treadmill and badu resistance jets (See INAQUA for details).
Q: WHAT IS THE ALTER G ANTI-GRAVITY TREADMILL?
A: The Alter G anti-gravity treadmill provides a walking environment with reduced weight bearing on the back, hips, knees
and ankles by airflow control in increments between 20% (weightless) to 100% (full body weight). This specialized
treadmill also has 3 video camera views for walking analysis and biofeedback training.
Q: WHAT INSURANCES DO YOU ACCEPT?
A: We are currently file insurance claims with Medicare, Medicare Advantage, Blue Cross and Blue Shield of SC,
Blue Choice, Tricare and Humana.
Q: HOW DO I FILE AN INSURANCE CLAIM THAT IS NOT IN YOUR NETWORK?
A: If we are not in network with your insurance company, our staff will gladly assist you with information on how
to file an individual claim. We offer discounted rates for payments processed at the time of service.
