INEVALUATION
At the initial visit, the client will return completed patient forms and a licensed physical therapist
will perform a thorough one-on-one physical therapy evaluation.
EVALUATION
Subjective Exam: Medical History, Complaints, Symptom Information, Patient Goals
Observation: Assess Functional Mobility, Inspect Soft Tissue Symmetry/Shape/Color/Mobility
Posture Analysis: Inspect Muscle/Joint Alignment, Posture Pro Digital Analysis
Gait Analysis: Analyze the Phases of Walking
Functional Assessment: Functional Testing, Assistance Level Rating
Palpation: Inspect Joint and Soft Tissue Mobility/Texture/Size/Temperature
Range Of Motion Assessment: Joint Motion Testing and Measurements
Muscular Assessment: Muscle Strength and Endurance Testing
Neurological Screening: Sensation and Reflex Testing
Special Testing: Additional Specific Tests and Measures indicated
ASSESSMENT & PLAN OF CARE
The physical therapist will establish individualized short-term goals, long-term goals and a plan of care
based on the evaluation findings and physician's orders. The physical therapist and physical therapy
assistant will execute and monitor treatments based on the individualized plan of care. The physical
therapist will routinely perform Re-Evaluations and alter treatments, goals or plan of care as needed
based on individual findings. An individualized patient progress report including physical findings,
treatment responses and assessments will be provided for the patient and physician as needed.

BILLING & CHARGES
Our insurance network includes Medicare, Medicare Advantage, Blue Cross & Blue Shield of SC, Blue Choice, Humana and Tricare.
We will also assist clients in filing individual claims upon request.
Clients will receive a complimentary discount with payment on or before the time of service.

Patients are required to cancel and/or reschedule appointments within 24 hours of the scheduled
appointment time to avoid a $50 cancellation fee per company policy.
